If you are planning to participate in a temporary food event such as a fair or festival, please fill out the temporary food event application form (below) and submit your application via email to This email address is being protected from spambots. You need JavaScript enabled to view it.. 

Organizations registered in Illinois as “Not for Profit” who present acceptable legal documentation to the health department and are serving food no more than three consecutive days or no more than twelve days annually need not pay.

Any applications received less than 7 days before the event is scheduled must pay a late fee of $25.00.
