Community Health Needs Assessment and Improvement Plan

The Kankakee County Health Department, as part of the Partnership for a Healthy Community, completes a comprehensive assessment of the health of Kankakee County residents using the MAPP (Mobilizing for Action through Planning and Partnership) process. The MAPP community assessments are conducted every three years, with the last one completed in 2021. Based on the priority issues identified through the MAPP process, action teams were created to conduct activities to address these issues.

For 2021 to 2024, these are the priority issues:

  1. Behavioral Health
  2. Health and Wellness
  3. Education and Employment
